City of Thornton, Telligen and Project Angel Heart are teaming up to provide a FREE 6-week workshop to help manage your diabetes. The 6-week evidence-based curriculum is designed to help gain a better understanding for people with pre-diabetes and existing diabetes. Caregivers are also welcome. Example topics include: healthy eating, balancing activity with relaxation, stress management, monitoring blood sugar, meal planning, managing medications and working with your health care team. These interactive classes are designed with group feedback, problem-solving, brainstorming sessions for challenges and solutions, and setting weekly action steps.Graduates will receive tools and resources to promote diabetes self-management.
Bonus: Currently Project Angel Heart is providing FREE daily home delivered meals to support your success during the 6-week self-management workshop! Each meal is designed with your health in mind and when combined with the workshop, may help you prevent and/or manage a diabetes diagnosis. Eligible participants for meal delivery must be Medicare recipients and have a current diagnosis of pre-diabetes or existing diabetes.
Project Angel Heart is a nonprofit organization that provides medically tailored meals for people living with life-challenging illnesses. Their team of professional chefs and registered dietitians use fresh, high-quality ingredients to prepare meals, which are then delivered to clients homes.
Telligen is a Medicare Quality Innovation Network/Quality Improvement Organization under contract with the Centers for Medicare & Medicaid Services. The organization is focused on improving quality of lives nationwide.
